LG-D500 T-mobile provider unlock done by octopus..[Reference]

Micro usb cable(5130)
Octopus LG Tool
Procedure
1.Turn on phone to normal mode
2.Ennable USB debugg
3.Connect phone to pc install drivers if needed
4.Run octopus select qualcomm platform
5.Select right model (D500)
6.Tick read NVM
7.Tick direct unlock wait for unlocking complete.
