sa mga nawala or nakalimutan puk code for smart user...
eto po solution......
1st log in muna kayo dito https://my.smart.com.ph/loginAuth
2nd mag add account kayo at ireg niyo ang number niyo na block
3rd then manage account niyo sim na iniregister niyo
4th go to INQUIRIES TAB sa left side
5th press PUK code inquiry
6th makita niyo yung ICCID
7th tanggalin ang sim at tignan ang 16 numbers sa likod ng sim mo at ilagay sa ICCID
8th then press check
done!! makita muna ang PUK CODE Mo
